What is the CFS Program?
The Coordinated Family Support (CFS) service is designed specifically for adults (ages 18 and older) with developmental disabilities who live with their families. It was developed in response to feedback from families who felt current services did not meet their needs.
CFS helps families:
Coordinate and schedule services they already receive
Plan for the future, including backup care and housing transitions
Access community resources, public benefits, and transportation
Train family members and caregivers to better support the individual

Who's Eligible?
Adults 18+ who:
- Have a qualifying developmental disability
- Receive services through a Regional Center
- Live at home with their family

What to Expect
Assessment (6–12 hours): A CFS provider meets with your family to explore your goals and needs.
Service Plan Development: Together, we create a personalized plan that identifies the supports you need.
Ongoing Support: You’ll receive wraparound services based on your needs—with flexibility as life evolves.
Check-ins & Feedback: You may be asked to complete brief surveys to help the state improve CFS services.
